Copy of Certificate of Registration of Business Name with DTI

For Manufacturer-copy of last year's production and sales report,
production capacities and copy of Environmental Compliance Certificate
(ECC), (if not yet submitted)

For Indentor-copy of contract/certification with manufacturer/supplier

For Bulk Handlers-list of clients, ports and bulk handling equipments,
owned and/or a lease contract

For Manufacturer and/Importer-list of distributors and dealers per
province by region

For Area Distributor-submit distributorship agreement/certification
from the suppliers and list of dealers

For Processor-copy of Mining Permit from the Bureau of Mines
and Geo-Sciences and last year's monthly production and sales report

Inspection report and recommendation from the FPA Provincial Officer

Warehouse registration for manufacturer, importer and distributor

Product registration of the fertilizer to be sold

License fee-based on company's capitalization. (Subscribed/Paid-up Capital
plus Retained Earnings).
